Coastal Home Health & Hospice – Mona Chandler (Mona.Chandler@coastalhhh.org)

  • Referral based (anyone can refer to hospice - physicians, family, neighbors) - services:

    • Home health (55 years in Curry County): services include wound care, in-home physical therapy, speech therapy - around 100 patients at any time

    • Hospice (27 years in Curry County): making patients as comfortable as possible as they transition - around 40-50 patients at any time

  • Serve Langlois to Brookings, also started to get clients from Del Norte

    • Applied for hospice licensing for Del Norte, office space in place, starting to recruit hospice nurses in Del Norte which has not had services for 20 years

  • Relocated to 585 5th Street, Brookings

    • Community room available for partner agencies - full kitchen

    • Durable medical equipment store (wheelchairs, hospital beds, etc), items covered by Medicare

  • Volunteer opportunities available, ranging from assisting with hospice, to recording stories of patients who are transitioning, walking patient dogs, etc - contact Mona to learn more

South Coast Hospice & Palliative Care – Doreen Kelly (d.kelly@schospice.org)

  • Hospice services provide comfort care and guidance to terminally ill patients and their loved ones

    • Patients create care plan in conjunction with patient’s physician, primary care nurse, social worker, CNA, chaplain, volunteer (if desired by patient)

      • Social workers help with basic needs (food, electricity), navigating social service systems, etc

    • Volunteers help with socialization and respite for caregivers - contact Doreen for volunteer opportunities

    • Care is covered by insurance or other means - no patient is turned away for lack of finances

  • Potential patients urged to engage earlier than later - proactive comfort care vs urgent care

  • Bereavement services offered to whole community, bereavement center is open again for group sessions

  • Care for the Caregiver services are being offered again

  • COVID impacts (past and present)

    • Helping loved ones learn about infectious diseases

    • Still doing COVID testing regularly

    • Huge nursing shortage - currently interviewing, patient referrals on hold temporarily while staffing levels are increased
